The inquiry committee constituted by the Health Ministry says that there were lapses in the treatment of Mehbooba Rehman Ankhi of Kashipur village in Nangalkot Upazila of Kumila.
The inquiry committee said that several doctors and central hospital officials were responsible for this, and the activities of the persons responsible and the organization were unethical and illegal.
Yakub Ali, the husband of the deceased, also had some liability in the incident.
The committee has mentioned a total of 14 lapses and illegal and unethical activities. Of these, 10 activities involved three physicians and four involved hospital officials.
Besides, Mehbooba’s husband Yakub Ali was also involved in four such activities, the probe report said.
