The 14-member committee, headed by Additional Deputy Commissioner Pallab Kumar Hazra, submitted the probe report to Deputy Commissioner (DC) Rahima Khatoon on Wednesday morning. Along with identifying the causes, the committee made 14 recommendations to avoid accidents on the expressway in future.
